Year Period	The program year the application covers.
Address	The address of the sponsor/SFA site.
City	The city in which the sponsor/SFA site is located.
State	The state in which the sponsor/SFA site is located.
Zip	The zip code of the sponsor/SFA site.
Ward	The ward in which the sponsor/SFA site is located.
Main Phone Number	The main phone number of the sponsor/SFA site.
